I've actually done this very conversion on a few 4004's, but I used a Ricknbacker tailpiece assembly, and a smaller version of that guard with four controls.

The original was cool - had a huge plastic spacer under the tailpiece made from pickguard material, and the original Maxima flatwounds from the 60's. 8)
