Except for the lack of a walnut-winged headstock, my 1975 4001S WHT is pretty Squire-ish:
I do remember seeing on eBay quite a while back a 4001S that had been converted to a CS look-alike with the addition of
painted-on "walnut" wings.

The glue on lower wing is drying right now and I expect to have some pics tonight. There were actually some rub marks where the neck slab scrapped against the lower wing - the source of the creaks. I used my jointer and took off less than 1/128" (if the gauge on the tool can be believed) from the face of each wing. The wood faces up very nicely now with no gaps.

I can see that the bass is now 1/64th of an inch narrower.

Ok, so the bass had BT hardware but none of it is in good shape, especially the tailpiece. I am thinking I would have the tailpiece powder coated or something and save it for another 5er project.
There is a plating shop near me that does automotive/motorcycle chroming (small batches), so I could have some of the totally beat hardware in my parts box replated. I was thinking about machining an old 4 string tailpiece to match the shape of the 5 string model and then have it re-chromed. Seeing as I am doing a CS look-alike, BT does not make any sense at all. What do you guys think?

Yes, chrome is the way to go for the look you want.

Yeah, if you're doing a CS look-alike you need chrome.
Nice work so far Ted.

The original surround is peeling a bit and it is chrome underneath. I may just remove the rest of the black and go with it as it is. Oh, and in case anyone is wondering, she is an '86.
