It looks like a custom finish 4001 series with different pickups. I'll let the experts look further.

It must have been donated before the rest of his bass collection to a swim.

I can't watch the vid as I'm at work, but if it's the black/white one its a later 4001 with a stripey refin,a Badass and 2 sets of EMG p-bass p/ups IIRC.

Nikki Sixx of Motley Crue also played a 4001. He had a set of EMG installed in it, The Hard Rock Cafe in Newport Beach has it now.

Apparently he still has a 150+ bass collection (has me beat by 141) which includes a late 60's 4005:

Well Alisha, here's the story as I remember it. There was a 4003 truss rod cover on Ebay a few years ago for sale. It was so specific in it's details that forum members surmised it to be from a particular limited edition 4003. An inquiry was made as to where the rest of the bass was and the seller informed those inquiring minds that it was in nonsalvageable condition due to water damage. The seller of the TRC went on to tell the story of how the bass got that way. The owner of the bass had a very large party at his house. Late in the evening the bass players wife went looking for her husband and found him passed out in a car in their driveway with another woman in a very compromising position. The now upset wife proceeded to toss all of said bass players collection of basses and some amps into their swimming pool. Somehow John Hall's name came up because he could confirm that the bass was unrepairable. His response was.."There are 5150 reasons why this bass can't be fixed". Now YOU figure out who he was talking about.
