S6 Lap Steel Name Plate wanted

I have a S6 lap steel that is missing the name plate. It's the older style T shape metallic name plate. Any clues on how to get one would be greatly appreciated.

That's a Model SD.
What you need is a flip-up headstock cover with a "Blade Logo" on it.
