Is it a direct fit? I'm hearing contradicting stories that the screw holes don't line up, and some saying that they do?

Any examples of oictures of 4003's and horseshoes?

I don't know the answer to your question for certain, but when I retro fitted my 4001 fretless with a horsie, I needed to bore new screw holes as the flared surround is a different size. The alternative is to install your horsie unit into the existing 4003 treble pickup surround, so the issue of whether or not the screw holes line up is effectively made redundant.

The pickup and surround fit fine, however, if you do not want to drill any holes in the bass the RI surround must be fitted 180º from it's normal orientation. You could use your existing surround to mount the pickup just fine.

These basses above, are they all the original mounts, as I'm guessing the horseshoes have the slant in the edges of the back of the mount?

Indeed, it'll fit just fine into the existing surround, as depicted here. As Jeff posted, if you wish to choose the vintage surround, new pilot holes will need to be drilled. I purchased my toaster and HS direct from RIC and placed them into this 'SnowGlo'. I absolutely love this bass.

Yes, the surrounds are original to those basses. The "slant in the edges" you refer to are what the '60s surrounds were shaped like, although you will find if you do enough looking around is that the shape varied quite a bit over the early years in particular.
