Someone correct me if I'm wrong, but I think it was the result of a scooter or bike accident. Paul grew his famous Sgt. Pepper-ear mustache to cover the scar on his mouth, and the other Beatles followed suit.

YIKES! I've never seen that picture of Paul! It's amazing, given the climate of the press today, that a Beatle could look like that and not have it end up in the news. I wonder, though - was the accident that chipped his tooth in the 'Rain' video the same accident that produced the scar on his lip that he covered with a mustache six months later? He looks pretty busted up in that picture. You'd think that if that was the accident that chipped his tooth, he'd have gotten his tooth fixed by the time all the cuts healed and they shot those videos. I don't see a scar on Pauls' lip in the 'Rain' or 'Paperback Writer' videos, especially one that he'd be self conscious of six months later. He doesn't have a scar on his lip, that I recall, in any of the photographs or videos of the summer 1966 tour. Did he have two scooter accidents during 1966? If this photo is from Autumn 1966, it must be one of the few photographs available of Paul from that time.

The always-credible Wikipedia references the Beatles Anthology Video #3 in this statement:
McCartney was injured in a moped accident on 26 December 1965, and closeups in the film reveal a scarred lip and a chipped tooth.[19][20] McCartney's appearance in the film played a role in the "Paul is dead" rumors from 1969. [22]

It's incredible, almost unbelievable, that a Beatle could have a substantially chipped front tooth and a scarred lip for six months in the middle of the 60s and yet there's so little photographic evidence. He had the money to go to a dentist, and he had the time since the Beatles were fairly inactive during the first four months of 1966... Maybe he wasn't so sure about dentists after John and George's experience!
