...should be a replica of this bass.

I'd rather it had a horseshoe and a tug bar. But everything else, down to the mono jack, is spot-on.

Hmm ... Wavy Grovers on a Sept '68? If anything they should be "Flat" Grovers. Also, probably the shortest guard I've seen for this time period? I can buy the green hi-gain though.
Hold-over from '68 finished later, perhaps?

This wasn't meant to say it doesn't look great though!

Wait a second, isn't "IH" an August '69?

You're right about the SN. This looks alot like the 69 that
Voltage has on gBase....
The late features are cool. If you look at this one and Graham's 21r
you can really see how the transition was made....
I do like how the copy reads on the "rare mono jack".
On an early 4001, ROS is the "rare" feature as it was not standard yet....

My 1993 4003S has this :D
My 2009 4003, no :(

Isn't that a crack on the neck at the 5th fret? Looks like some jack tried to tighten the truss rods like on a modern bass.

Odd place to crack though. Doesn't look to bad to me, assuming the rods still work properly.
